The 95% to hit is as best as you can get in Fallout 1/2, IIRC. The 5% failure is for events that are classified as "shit happens". [img]graemlins/hehe.gif[/img]

Not sure about The Slayer Perk. I am more of a "sniper" and "bonus range damage" person.
