In order to gain skill in sneaking, you need to be near a creature or npc and sneak undetected: I once tried to sneak from balmore to gnaar mok (no, I'm not crazy: with boots of blinding speed sneaking is as fast as normal running). My sneak skill didn't increase at all (it was something like 23/100 and it remained still, I checked). Then I got near a mudcrab, and suddenly it increased like to 90/100.
Which means that sneaking alone is useless, only sneaking around somebody/something able to see you does help, and the longer you stay in sneak mode without interruptions, the more your skill increases.
A good way to increase without training is this:
get near an NPC, where he cen't see you easily
put a weight on your sneaking key and go live your life.
Come back after a while and your skill has increased like 50pts.
sleep and get that int x5 multiplier.
Note that if sneak is minor/major skill, this tactics is highly detrimental: you will gain 10 levels at once, but will lose all the multiplier bonuses for all the levels you get after the first one.
