Yes the patch is worth it. No, it doesn't screw up your old saves. If you get some "error" message related to your save game, just click "yes" and you should get through to your game. Once you re-save, you shouldn't get the error the next time around. You may also get "error" messages about your plug-ins. Also click yes and don't worry about it.
Note that the European 1.2.something patch is now (sorta) available. See my other post.
