In that respect, BG2 is quite realistic. If you appear in town as a raving, deadly sociopath, the local forces will try to put you down immediately. Kill a few, and you'll have a rock bottom rep--which means the news about your activities will in effect travel far and wide, causing merchants to shun you, and armed forces to seek you out.
