if you have Jaheira in your party when you find Khalids body in the initial dungeon, she suggests that there is a point beyond which the damage to a body makes resurrection impossible.
If a person is hit by a disintigrate spell or smashed for so much damage that they burst into chunks then they're gone for good.
Other effects that make a portrait disappear (flesh to stone, imprisonment, etc) can usually be reversed, one way or another.
