The Deathknight room is located in the Kuo-Toa lair in the Underdark (far western side of the underdark map). It's the easternmost room in the lair.

To get the Death Knights to appear, you have to:

make a living sacrifice on the altar. BEST BET: summon something and lure it onto the altar. "Spider Spawn" comes in great here.
