if you are in a romance then your lover will be targetted by the bad guys and they will swing him/her to their way of thinking. (Sorry dude, I dont want to spoil too much.) You need to collect their sorry carcass (and items) and try to save their soul. It doesn't affect the game one jot, it's just a small diversion for those in a romance.
