Crom Faeyr: the scroll is on the body of the Shadow Dragon in the Temple Ruins, the belt is in the Kuo Tuan Dungeon in the Underdark, the Gauntlets are inthe Planar Sphere, and the Hammer itself is in the illithid lair in the Sewers of the Temple District.
