There was an example in one of my sourcebooks (Greyhawk Adventures, I think) of a good wizard who DIDN'T KNOW he'd become a lich. The process normally used needs someone evil to do it (Werewolf and Vampire body parts, become Wight until rest of process finished, etc.) I believe the process was originally published by Dragon magazine years ago
