With HOTU the system requirements have been upgraded - you basically need a slightly better system than that required for NWN's or SOU. What video card do you have? (You can find this in the display properties on your computer settings or see it come up briefly as your PC boots when you turn it on.
Make sure you check the sound settings in the game (Options - Sound Options - Advanced Options) and put it on Miles Fast 2d Positional Audio. If that does not help write back here with as much info on your system specs as possible and we'll see what we can do.
