Hmm that could be it.
If you have, say, the axe + shield equipped, then you ask for the axe + hammer, it searches your inventory for the axe. Maybe, since it can't find it in your inventory (assuming it doesn't search equipped slots) it assumes you no longer have it and erases it from your quickslots.
I really have no idea whether that's anywhere near right, but it's just a guess.
