The five elements are:
Face of Corruption - next to the skeleton which is holding onto the chain
Idol of Darkness - after the chain puzzle
Paw of Contagion - given by Ardibren
Tusk of Lust - from Lysandra
Light of Death - in D'Soto's bedroom
I'm not sure which three you have but it sounds like you are missing the last two. Lysandra can be reached by going down to the catacombs where you meet the zombies, go to the last cell, drop down the hole, go through the sewers,past Azgorax, pull the chain, back down to the sewers to take the east exit and have an interesting encounter
D'Soto's room is after Lysandra and you follow the yellow brick road..
