Okay, I'm no expert in this so I recommend that you cross post this over to Saz's techie help desk thread so the real gurus can help you out.
DirectX 8 might be your problem. I personally am able to run this game with DirectX 8 but that seems to be the trouble spot for many other systems. Check out the Best Tips thread for information on how to downgrade (I know, it's a drag) to DirectX 7.
There should be something in Saz's thread or in the Best Tips thread that directly relates to problems with entering or exiting town. I hope you find the solution to your problem there.
If not - do let us know and we'll try to figure out what is going wrong.
