Some stairs can only be approached from one side. In toad village IIRC you have to walk past the ladder, trun around, and face it from the other side. Then look up, and go forward. I personally hate ladders the way they are done in this game, but with a little patience you can climb up. Going down is another story. I simply take the 1-5 points of damage and jump.
