Search everywhere. It is just lying around somewhere. I ended up with two empty flasks. Kept the extra one just in case until I was sure I didn't need it.
When you reach the point where you will use the flask, do not enter (fall) into the crusher. I thought I could walk out of there but couldn't and had to go back to a saved game.
