As I vaguely recall, you need a Dwarf (I think the dark Dwarf kind) to make something on the anvil. Never had one, so can't say if what he makes is worthwhile or not.
The main thing in the Flayer lair is to get out, either by killing the big brain or intimidating it into letting you leave freely without a fight.
