The Altar is behind the "Embrace the Shadow" door (the one with the actual shadow on it, as opposed to the one that says "Embrace the Shadow" in the description).
To open the door, you have to use the Lightning Prism, but the outer door (that says "Embrace the Shadow" in the description) must also be open for the bolt to hit the inner door that leads to the Altar and "Auril".
The only way to open the outer door (afaik) is to go to the Portrait/Teleport thingamajig on the staircase and choose "Lysan". This will teleport you to the anteroom between the two doors.
Send one character to stand next to the outer door (leading to the hall). Send a strong character to stand on the shadow covering the inner door ("embrace the shadow"). This character will take damage for as long as they are standing on the shadow (possible exception if the character is evil, but I never tried that), so open the outer door quickly so that everyone can exit. The idea, if this is unclear, is that the outer door can only be opened when someone is standing on the shadow covering the inner door.
Then head back to the Lightning Prism and point it so that the bolt ricochets around to the Shadow door (you probably have to move at least one mirror inbetween, but it's not hard to figure out), then fire the Prism.
As for how to destroy the Altar, talk to the White Aabishai (when fully healed so they don't eat you) on the first floor, then talk to Nickedemus in his bedroom next to the Lightning Prism.
