As far as I know, the base number of attacks per round -for a fighter at least, I'm not sure about monks- will never exceed 4. The only way to get 5 is, as you said, by casting haste, or when dual-wielding or using the rapid shot-feat. That's the way it's supposed to be, to my best knowledge.
