The closest they've come to what you're wanting is Neverwinter Nights. If you have a friend/acquaintance who has the game (and you both have broadband connections to the net) one of you can be the DM and create everything from scratch. It's *almost* like pen and paper D&D. Creating what you're wanting may actually be counterproductive for TSR. I also believe it is beyond the capabilites of a standard PC game. I know there are Massively Multiplayer Games that exist online (Ultima Online) that might come close to what you're looking for. Maybe TSR is moving toward that with Neverwinter Nights.
