In answer to your questions...

1) Secret door
I'm not entirely sure which secret door you are talking about. Are you talking about the one in the "puzzle room"? If so... look for spoilers at the bottom of this post.

2) Razorvine Extract
If you retrieve the razorvine extract before talking to Bandoth, you cannot give it to him. It is a bug. But he only gives some XP anyway, so it's not going to ruin your game.

3) The Severed Hand Quest
The quest items (seeds, pure water, animals, and birds) cannot be found until farther along in the game. However, it's worth completing the quest, because it gives quite a sizeable chunk of XP.

Look at the table in the room preceding the puzzle room. It has three symbols. Go back into the puzzle room, and walk only onto the corresponding symbols, in the order they are on the table. Then go back into the table room, and look for a statute with its hammer held up. Pull it down, then go back into the puzzle room. There should be a staircase in the middle of the puzzle.
