Is the order you've listed above the order in which you installed things?

If you installed the 1.41 patch after installing TOL, that is quite possibly your problem. TOL itself acts as a more recent patch to HOW. Applying the 1.41 patch after TOL could leave you with a buggy and unstable installation.
