It's not a bug in your game configuration, EvilColm, just a poor job of attention to detail on the game designers' part. After you rescue her, she remains near the entrance to the next level in cae you need healing. After you complete the Dragon's Eye area, however, she will simply disappear without reappearing anywhere else in the game.
The problem with Orrick, however, is a known bug and was fixed in one of the official patches.
