You have a +5 to hit, and a +6 damage.

The STR 18 is +1 to hit (THAC0), and +2 damage, and the Cairn Blade is +4 to hit and damage.

This doesn't mean your damage range is 7 to 16; there's still critical hits to calculate.

Also, the Cairn Blade is a 2-handed sword, which only does 1-10 damage against small and medium creatures. In normal D&D, the 2-handed sword does 3-18 damage versus large creatures, like dragons and ogres and trolls (oh, my!). I don't know if IWD follows this rule; the damage listed for every weapon in the game is only against small/medium creatures.
