Not sure if you mentioned that you did this or not in your post, but have you tried uninstalling, installing vanilla + patch + fixpack, then starting a new ToB game (not importing your saves, which I assume you are doing?)
perhaps its a corrupt save file, or something.
(By all means I have no expertise in this matter, just aiming on possibilities
