Two additions, and a few errata.

Disintegrate.

In the "Test of the Spirit" section of Watcher's Keep, if the Spirit Warrior dies, so do you.

Betraying the SThieves in Chapter 3 doesn't necessarily kill you, it just makes it impossible to continue in the game. Arkanis Gath can be "beaten," but since siding with Aran makes an enemy out of Bodhi, you're stuck, unless you're playing with some mod that adds a 3rd route to Spellhold.

Any successful Vorpal hit will kill you, whether it comes from your own party or not. (The Glabrezu in ToB are very dangerous for this factor alone.)

Demilich Howl should be grouped with the other death spells.

I'm not sure, but I think the harmful side effect of using a certain ability for too long doesn't kill you outright, it simply does magical damage. Have somebody cast PfME on you and you may be good to go--but I hardly ever use this ability, so I'm not sure.

And I think it's not simply "Whole party charmed, or Whole party mazed," but rather "Every party member is either Charmed, Mazed, or dead."
