Is your inventory full most of the time? If you gain items (especially through a dialog with someone) and there aren't enough inventory spaces, your character will drop an item out of the 1st few slots. If you happen to have the volume down and can't hear your player say "I had to drop the item" or the text box goes by too fast to notice the "Inventory full: Item Dropped" line, you might accidentally leave behind something like an ammo belt. This exact scenario happened to me, except it was my Bag of Holding and I was unable to find it again. Boy was I peeved.
