Jaheira - fireballed her to death in her cage at Chateau Irenicus.

Minsc - ditto.

Nalia - Had her Polymorph Self into a sword spider inside the D'Arnise hold, then went outside, thus changing areas. For some reason this kept her as a sword spider permanently. Then I ditched her at the Copper Coronet, where she is now one of their most *ahem* interesting patrons.
