No, there's only a 5% chance of a wild surge happening. Unless you cast Nahal's Reckless Dweomer, then it always happens.

No, only when the effect warrants a saving throw. For example, if you get the wild surge effect 'caster held', then your character must save vs spells, if he fails he is held, if he is successful, there's no effect. Beneficial effects never give a saving throw.

I don't think so. I haven't had Keldorn and Sarevok in the same party ever, so I'm not 100% sure, but I have never heard of it happening.
