if you remember, the game auto-saves under Final-Save or something like that, when you use the first Tear of Bhaal. the best thing to do is to load that savegame, replay the final battle with Johnny and then, after all the ending ceremonies and animations, you will be taken straight into ToB; with all your equipment.
you can also import your party from any saved SoA game directly into ToB, but that would mean you'd be giving up all the cool bonuses you get from the Tears of Bhaal.
