only some of the dialogs are triggered by real time. most are place or event based since ToB is far more linear than SoA. once youre able to access a new area, hang out there a while and see if anything pops up. for example

in the temple of bhaal after you kill "gorion", you get a new romance dialog regardless of how long you were there. same with killing yaga-shura (at least with viconia).
