I've just found out what caused it, and it doesn't involve her dying.

just do the "remove fatigue" trick

: remove her from your party, but dont actually let her leave when she asks "are you sure?"

Limitless symbols available this way, each worth 2750 gold, although collecting gold isnt really a problem (but spending it seems to be)
