Believe me, this is a bug I've wanted to quash for a loooooong time, as I experienced this in almost one of my first games. Typically, re-equipping the shield or reloading will get it to start reflecting once again, though sometimes you have to be persistent.

There's nothing wrong with the item file, which makes me suspect that the physical mirror opcode itself is flaky (or has some as-yet-unknown parameter). All of the items which use this opcode--Shield of Balduran, Reflection Shield, and the Cloak of Reflection--all seem to share this issue. I'm not enough of a spell slinger to know if the Physical Mirror spell itself shares the same flakiness.
