Answers (spoilers)
1. List all the possible ways for the PC to die (anything which results in game-over means death, for the purposes of this question). (Also, health reaching 0 only counts as one way of dying - Don't list every possible weapon, trap or spell that can reduce the PCs health to 0.)
*Health reaches 0
*Petrification
*Imprisonment
*Str drain to 0 from undead
*Int drain to 0 from Mind Flayers
*Cha drain to 0 from ugly stick (Rod of Terror)
*Level drained to 0
*Vorpal hit from own party member
*Betraying the Shadow thieves, having allied with them
*Betraying Bodhi, having allied with her
*Taking the Rift device from the sewers
*Demi Lich Howl
*Dire Charmed type effect while solo
*Maze while solo
*Finger of death, Power word kill etc... Instant death spells
*Going to fight Irenicus in spellhold without help
2. List all the characters who must be killed by the party in battle in order to win the game.
*Irenicus
*Bodhi
*Kobold shaman with crystal piece
*Dace
*Spore colony in spellhold
*Tree of life parasites
*Tree of life parasite guardians
*Wraith Sarevok
3. List all the characters who must die in order to reach the end of the game but don't have to be killed by the party.
*Shadow thieves who get wasted by Irenicus in and around his dungeon
*Cowled wizards who get wasted by Irenicus in and around his dungeon
*Mook
*Cowled wizards killed in cutscene where Irenicus escapes
*Shadow thieves killed in Irenicus's Bhaalspawnness extraction.
*Dradeel
*Tiax
*Wanev
*Aphril
*Njal Skal (only one point if you just said 'the spellhold inmates' which isn't entirely correct)
*Elves Irenicus kills in cutscene in underdark
*Creatures in Rilifane cutscene
*Elf and Demon in cutscene
*Yoshimo (An issue of contention for two reasons. You can bug exploit to keep him, and if you never go to the coronet after leaving him in Athklata, you won't see him die. However, it is made clear that his death is a certain result of the geas spell he is under and his death is part of the storyline, therefore I am including him as an answer to this question).
Common wrong answers
2. Assassin in Brynlaw (who can be avoided altogether), Irenicus's Black Dragon (which can be persuaded)
3. Anyone who dies in a dream sequence on resting (you could get through the game without resting), Phaere and Ardeluce (you can get out of the underdark without doing the scrambled eggs quest), Dill (who doesn't actually die with the other spellhold inmates)
