That's how you change your main character's portrait. Changing the pics for the NPCs is more complicated.

You might be able to do it with GateKeeper, but I'm not sure. I know for a fact that you can do it by renaming files: Coran has a truly butt-ugly picture, so let's say you want to change Coran's potrait. Find a Baldur's Gate (or IWD) picture of a good-looking male Elven warrrior, and put the files in your Portraits directory with the names CoranL.bmp and CoranS.bmp. I don't think the capitalization is too important. Anyway, when you restart the game, Coran will be using the new pictures--the files in the Portraits directory will override the ugly mug he usually gets. If you want to change back, just delete, remove, or rename the Coran bitmaps. I actually found this out by accident when I decided a portrait really reminded me of Imoen. Pretty cool.
