OK, firstly: installing Rogue Rebalancing and Tactics is the same process as installing UB. Secondly, the problem seems to be that you have installed the patch AFTER UB. If this is the case, run Setup-UB.exe again, and press 'r' to reinstall everything.

EDIT: As for the Spellhold dream bug, make sure that you do not touch Bhaal at all until Imoen says her line ("Now it is in my sight. He has shown me how to make it vulnarable!"). If you hit him before then, I have known exactly what you describe to happen.
