I had the exact same bug, and never found a solution. I went through the Tactics mod with 4 separate parties, and with every single one (and on every single Reload) found his Lich form to be unkillable. In the end, I had to CLUA in a scroll of Shapechange, and his DemiLich form.

default installation, I can only say that even
my computer isn't that buggy.
