The "Spell Sequencer/Contingency Icon present, but no Abilities button" issue is a known bug, that has no real fix as far as I know.
Workaround Solution: Ctrl-Y, Ctrl-R. You'll have to Rest again to restore the extra spellslots granted by items, but that's better than never being able to cast Contingencies again.
