Only thing I can add is that the recipe book and the item to be enchanted have to be in the same inventory.

If all the other conditions are met, left click on the recipe and you should get a coloured (purple to my eyes) symbol which you can drag to and click on the item to be enchanted.

Not all casters can enchant. Warlocks have to take another feat (imbue?) and I think there may be problems with class based casters such as Spirit Shaman.
