Arkanis will show up and whack you if these are both true.....
- It's Chapter 2 or 3 (or it's Chapter 6 or 7 and you haven't patched your game)
- You refused (or haven't yet accepted) the quest with the woman in the Graveyard District

And then one of these becomes true.....
- Anybody in (or just outside) the main Shadow Thief building (the one with Renal Bloodscalp) turns Hostile to you
- Anybody in the secondary Shadow Thief building (the one with Mae'Var) turns Hostile to you, unless you have been specifically instructed to make them Hostile
