Your best bet is to start with an item as close to your intended custom item as possible. For instance, if you wanted to make a Ring that could cast Lighting Bolt, a good place to start would be the Ring of Energy. What I do is I open up Shadowkeeper's Item Browser, find the item I want to start from, and say "Extract Item." This puts a copy of the ITM file in the Override folder, and I can then use IEEP to tinker with it to my heart's content.
