If you're trying to get to the ToSC areas in a game that you started BEFORE you installed ToSC, you'll need to run the MCONVERT program that's in your Baldur's Gate directory. This will update your "map" files so that Ulgoth's and Durlag's are visible--and reachable. I had a beast of a time trying to figure this out myself.

You only need to run MCONVERT to update an existing game. All games that you start after installing ToSC will run normally.
