Dr. Doom is certainly the heaviest hitter on the list above when it comes to pure, sane evil.
The Super-Skrull also comes to mind, but that's the one from the 60's, 70's, and (early to mid)80's because I don't know if they "wussed him up" or not lately....lol.
From the same time frame there's also the Red Skull, Baron Zemo, Kingpin, Ultron, and who could forget the pure evil that was...Dark Phoenix?
Sabretooth actually came on the scene in the late 70's, but he was a two-bit villain prior to his re-vamping in the mid-80's and wouldn't even come close to your list back then. Actually, in those days, he would have qualified for the lamest villain list, but now I'd say he makes it into the really evil club. At least as of the mid 90's, for all I know he's a member of the X-Men now...lol.

I loved the Super Skrull as well. He was a wicked kickazz baddie, awesome when drawn by John Byrne.
Sabretooth did start out as a twobit flunkie in PowerMan & Iron Fist. But i totally agree with you that he may not be a great one.. but for sure one of the better
evil ones.
Kang was always one of my faves including all his different incarnations, Immortus, Rama-Tut, and im sure their are some others their as well.
Ultron rocks as well. What a wicked villan.
Mephisto was another great and powerful villian that i loved to hate.
