It'd be similar with Dick and Bob:
Richard = shorten to Rich, pronounce with a hard 'ch', and voila - and I think that some older writing styles have 'D's and 'R's looking exactly the same.
Bob would be exactly the same story as Bill - shorten it, apply an accent which changes the initial sound, and voila. \

Jack from John - maybe they thought that "this is the house that John built" didn't have the same ring to it?
