I'm trying to remember can I swap my henchy any time during any chapter apart from Chap. 1 at Helms Hold?

No, I don't think you can even take a henchman to Helm's Hold.

Are they present during the ceremony? I seem to recall running that portion solo. However, it's been quite a few days since I've even looked at the OC.

Yes, you can take your henchman to Helm's hold, and no you can't swap him.

And answering your original question, you can't swap your henchman in chapter 4 either.

It will make whatever henchman that you choose that much more effective. And I believe you can swap the henchmen in the first three chapters, but not the fourth one. I haven't played the game in a while, but I believe during the second chapter you can swap your henchmen in and out throughout most of the scenario.
