Quite terrible. At the same time, it may be a hoax. Otherwise I think there's some serious privacy issues at hand here, since the student's name (Michael Benson) and his Student Number (usually the Social Security Number) are clearly marked on the test form. If it's legit, can you say Identify Theft, kiddies?
