The hilt of the Equaliser?

I had the Crom Faeyr scroll on my main character but playing again i can't find it 2nd time around.

Equaliser parts: pommel in Irenicus' dungeon, hilt in illithid city in Underdark, blade in beholder city in Underdark (these last two could be vice versa, I'm not sure)
Crom Faeyr: the scroll is on the body of the Shadow Dragon in the Temple Ruins, the belt is in the Kuo Tuan Dungeon in the Underdark, the Gauntlets are inthe Planar Sphere, and the Hammer itself is in the illithid lair in the Sewers of the Temple District.
