The Girdle of Frost Giant Strenght(i think) is in the Ku-Toa dungeon in the Underdark. It is being worn by a Demon Knight.

The Hammer is in the Mini-Mind Flayer lair in the sewers under the temple district.

You can only get the demon knights to spawn by summoning animals under the Gorgaon Statue in the Kuo-Toa Dungeon in Underdark. And for the mindflayer dungeon under Temple District, you will need the sewer key from Tazok in Windspear Hill to enter legitly.
