I've always played through the game with good characters. Is it worthwhile to go through it evil way, or is it pretty much the same? Same ending?

The ending is the same, you only get different dialogue scripts with certain NPCs. Playing evil con be consider fun and sadistic at times, just make sure you distinguish between evil and psychotic.
