I just got out of the Underdark...but when I try and go back to areas from disk 3 or 4...it says wrong disk (so I enter the disk it asks me to...2 or 3), then goes to a blue screen saying 'Media Removed'...and wants disk 4 back in. It goes back and forth like that a couple times...then just does nothing.

Umm...I have the latest patch installed....any ideas?

Did you try doing a full install? If you can do that, you won't need to switch disks during the game.

You might want to save your "Saved Game" files to a floppy disk or to a different part of your hard drive before doing the full install -- just in case.
