At level up. My fighter has 19 con; how do I determine what the maximum hp he can obtain? What do I do with the d10?

d10 + 5 per level up until level 11, IIRC.

Level 10 is the first level in which you gain the streamlined HPs. Levels 1-9 will be 1d10+5 HPs, for a maximum of 135 HPs, and 3 HPs per level after that.
